Accessing Your Health Record

Your Right to Access Your Records
Under Ontario’s Personal Health Information Protection Act (PHIPA, 2004), you have a legal right to view or request a copy of your own hospital health record.

Listowel Wingham Hospitals Alliance is committed to keeping your personal health information safe and confidential.

This section explains how to access hospital-based health records from Listowel Wingham Hospitals only. If you are looking for records from your family doctor or another hospital, please see Option 4 below.

Option 1: Request Hospital Records Directly from Health Records

You can request a copy of your Listowel Wingham Hospital Health Records by contacting the Health Records Department directly.

  1. Complete the appropriate form:

    These are fillable PDFs. Save them to your computer to fill out electronically, or print to complete by hand.

  2. Submit the completed and signed form by email, fax, or in person, along with photo ID for the patient.
    Note: Requests for anyone other than the patient require a valid legal directive (e.g., a Will).
  3. We have up to 30 days to process your request after submission.
  4. Fees:
    – $30 + tax for the first 20 pages
    – $0.25 per page for each additional page
    Invoices must be paid before the records will be released. Payment options: in person, by phone, or on our website.
  5. Receive your records via paper or electronic format (based on your preference indicated on the form).
    A Health Records staff member will contact you once your records are ready.

Option 2: Online Patient Portals

A) ConnectMyHealth (Free)

ConnectMyHealth is a secure online portal giving you access to lab results, diagnostic imaging, and select electronic reports from participating hospitals in Southwestern Ontario, including Listowel Wingham Hospitals.

  • Free to use
  • Valid OHIP card required
  • Photo ID and OHIP card must be submitted during sign-up
  • One-time registration with reusable login
  • Operated independently from the hospital—contact ConnectMyHealth for support
B) MyCarePortal (Coming Fall 2025)

More details will be available closer to launch.

C) Other Patient Portals

If you received care at another hospital, please visit that hospital’s website to learn which patient portal they support.

Option 3: PocketHealth (Images Only)

PocketHealth is a third-party platform that allows you to view medical imaging only (e.g., X-rays, MRIs). Reports are not included.

  • Subscription required (various pricing plans available)
  • Operated independently from the hospital
  • Contact PocketHealth directly for technical or billing support

Option 4: Records from Your Family Doctor

For health information related to visits at your family doctor’s clinic, please contact the doctor’s office directly. These records are not stored or accessible through the hospital.

To provide you with quality care, the hospital collects both personal and health information about you. This includes:

  • Personal Information: Legal name, date of birth, address, email, health card number, extended insurance numbers
  • Health Information: Medical history, details of hospital visits, diagnoses, treatments, and services received during those visits

A health record is comprised of:

  • Personal information (e.g., name, date of birth, gender, address, and extended health insurance numbers).
  • Personal health information (e.g., Health Card Number, information relating to previous health problems, diagnosis, the record of your visits to the hospital, and what health care we provide to you during those visits).

This information can be gathered and stored in several ways:

  • A hard copy hospital chart maintained by Health Records
  • An electronic patient record
  • Documents created and stored in patient care areas and private offices
  • Diagnostic images and reports
  • Lab specimens and reports
  • Photographs
  • Videos

We collect your personal health information to support high-quality care and improve health outcomes. Your information may be used:

  • To provide and coordinate your hospital care and any necessary follow-up in the community
  • To carry out quality assurance activities to improve patient care
  • For patient satisfaction surveys that help us evaluate and improve services
  • To meet legal and regulatory requirements
  • For research, to improve care and develop new treatments and technologies
  • For education and training of current and future health care professionals

For Another Individual:

Consent for the collection, use, and/or disclosure of personal health information is obtained from the patient, regardless of age, if the patient is capable of understanding the information presented to them and the impact of consenting or declining. If the patient is not capable, consent is obtained from the Substitute Decision Maker (SDM). The SDM may be:

  • Guardian of the person or property
  • Attorney for personal care or property
  • A representative appointed by the Consent and Capacity Board
  • Spouse or partner
  • Child or parent
  • Parent with right of access
  • Sibling or other relative

In case of conflict, the capable patient’s decision prevails with respect to consent for collection, use, and/or disclosure.

For Other Health Care Providers:

When requested, copies of your health record may be released to health care providers outside the hospital to ensure the best continuing care for you. Your attending physician at the hospital may also share reports or summaries of your treatment at the hospital with other physicians and health care providers involved in your care to ensure they are aware of treatments or medications that may affect your ongoing care.

For a Deceased Individual:

If you are requesting a copy of the hospital record of a deceased patient, you must submit proof of your legal signing authority and complete the Release of Information form. Acceptable documentation includes:

  • A copy of the deceased person’s Will naming you as the Executor
  • OR a notarized letter from a lawyer confirming your legal signing authority (if no Will exists)

Wingham Emerg Door Replacement

Due to the replacement of the Emergency Entrance Doors, on Wednesday October 23rd, all Emerg Patients will be directed to the Ambulance Doors around the corner to the left of the Emerg doors.

 

Due to the replacement of the Main Entrance Doors, on Thursday October 24th, anyone using the Main Entrance will be directed to the Emerg Doors to the left of the Main Entrance doors.
